This is a Nintendo Game & Watch Mario's Cement Factory from the Wide Screen series, released by Nintendo Co. Ltd. in 1983. This box is the "Pocketsize" version, produced in limited numbers by Nintendo of America in the early 1980s.

Comes with everything pictured. Batteries not included.
